Once you have registered with Functional Medicine of Idaho, please begin your New Patient Intake forms listed under the “Questionnaire” tab.
After intake forms are complete, please upload a copy of your insurance card (front and back) under the “Labs and Documents” tab. This page will allow you to easily take a photo of your insurance card with your phone, and upload.
Our team of providers is in network with most Blue Cross, Regence, Aetna, Select Health, Pacific Source, Cigna, and Mountain Health Co-op plans.
Out of Network Information: Most insurances not listed above are considered out-of-network. Out of network patients are considered self pay although FMI can provide you with a detailed bill for most services that contain ICD-10 diagnostic codes and CPT codes for reimbursement from your insurance. We accept all HSA and FSA cards as a form of payment.

Functional Medicine of Idaho is NOT contracted with Medicare and Medicaid.
Medicare and Medicaid patients are consider self-pay patients.
It’s important to note that Medicare and Medicaid will not reimburse for care at FMI.

Yes, we are pleased to offer the convenience of accepting Health Savings Account (HSA) and Flexible Spending Account (FSA) cards as valid forms of payment. It is important to note, however, that the coverage of the FMI Functional Care Plan by your HSA or FSA is subject to the policies and guidelines set forth by your respective healthcare financial account. We recommend you first verify the eligibility and coverage details with your HSA or FSA provider to ensure a seamless and informed payment process!

Every insurance plan is different. To help you determine what your plan will cover at our office, we have put together a list of common appointment, lab and diagnostic codes used for billing purposes. We are more than happy to assist you with any inquiries related to CPT codes.
If your plan is in-network with Functional Medicine of Idaho, an FMI team member will be able to verify your eligibility and assist you with the following:
Yes, payment is required at the time of service for all co-pays, deductible retainer (if applicable), and any outstanding account balances. Deductible retainer is $150 retainer for all patients who have not yet reached their deductible. This retainer is applied to the total cost of the appointment. Any balance after claim is processed through insurance and retainer is applied, will be billed to the patient. Deductible retainer is not collected for most Wellness Appointments, but varies by insurance policies.
Labs are NOT included in the FMI Functional Care Plan. This allows patients the freedom and choice to choose the lab they would like to be drawn at. FMI utilizes 3rd party labs for lab-draws. To find out which labs are in-network with your insurance plan, call the member service number on the back of your insurance card, or log in to your insurance provider’s member portal to find out which lab is preferred.

Your functional care plan can be discontinued at any time. The fee of the functional care plan is for the creation of your individualized care plan that is crafted specifically for you by your functional care provider. With this, the fee for this plan is nonrefundable.
